COVID-19 Office Policies
**Please bring proof of your COVID vaccine if you are vaccinated. We can make a copy and scan it into your permanent chart. This way, if you lose your card, you have a backup in your medical chart. Please wear a mask upon entering our office and upon COVID vaccination verification, we will give you the option to stay masked or to unmask.
